This confidential and anonymous system makes it easy to report an incident about workplace issues like financial and auditing concerns, harassment, theft, substance abuse and unsafe conditions.

First, find your organization

Begin typing the name of your organization below. After you select the organization we’ll ask you a few questions about the incident.

Have you already made a report?

You can communicate with the organization and check the status of your report using the access number and the password you created when you submitted your report. You are encouraged to check back regularly to view messages and the status updates in your report. If you need assistance or forgot your access number please call us. For the United States or Canada call (800) 461-9330. If you are located in any other country please call the international number provided by your organization.

Will this remain confidential and anonymous?

Depending on local laws and the organization you are submitting the report to, anonymity options may vary. In all instances, every effort will be made to ensure that information relating to a reported incident is kept confidential and communicated on a need-to-know basis only. Below are the different levels of anonymity you may be able to select depending on the organization you choose:

  1. Remain completely anonymous
    You will not reveal your name or contact information to your organization or to Convercent. Your identity is completely protected on the incident report.
  2. Remain anonymous toward organization
    You are comfortable revealing your name and contact information to Convercent, but not to your organization. Convercent may contact you confidentially to gather additional information about the report, yet will not reveal your identity at any time to your organization.
  3. Share my name and information
    You do not care about anonymity. You choose to reveal your name and identity to both your organization and to Convercent.

Can I change my report or check its status after I submit it?

You'll be able to create a personal password, which you can use to check the status and updates of the report after it's been submitted. You won't be able to change it but you will be able to add additional details using message boards.

Who will actually see the incident report?

After you submit it, your incident report will be received by Convercent and sent to the individuals deemed appropriate within your organization. Your organization handles all incident reports according to its internal investigatory procedures.

Will my incident report be shown to the police?

Your incident report is NOT automatically sent to law enforcement. Only if your organization feels law enforcement should receive a copy, or if the law requires it, will your incident report be submitted to the police.

What kinds of incidents can I report on?

Your organization provides a list of incident types that it recommends you choose for your report and you always have the option of typing in your own incident type.
